Patient Portal Terms
These terms govern use of the portal where you view information your nutrition clinic shares with you.
What the portal is
The patient portal is a supper service that lets you, when your clinic enables it, view nutrition follow-up information (for example appointments, consultations, or other data your clinic chooses to share).
Your nutrition clinic manages your care relationship. Supper provides the technology platform. By using the portal, you accept these terms and the Patient Portal Privacy Policy.
Acceptance
Accessing or using the portal means you accept these terms. If you disagree, do not use the portal. We may update these terms by posting the current version on this page; continued use after a material change may require a new acceptance inside the portal.
Access
Access is usually granted via a link sent to your email or other mechanisms supper or your clinic enable. You must keep that link confidential and not share it. If you suspect unauthorized use, contact your clinic and support@thesupper.co.
Acceptable use
Use the portal only to view your own information and take actions the portal allows (for example confirming an appointment). You must not attempt to access other people’s data, interfere with the service, or use the portal unlawfully or abusively.
Availability
The portal is provided “as is” and “as available”. We do not guarantee uninterrupted or error-free service. Your clinic may enable or disable the portal.
Clinical content
Clinical or follow-up information you see in the portal is your clinic’s responsibility. Supper does not provide healthcare and does not replace advice from your professional.
Limitation
To the extent permitted by applicable law, supper is not liable for indirect damages, lost profits, or data loss arising from use of or inability to use the portal. Any direct liability attributable to supper will be limited to the fullest extent permitted by law.
